Coming from the South:
Take Interstate 271 North to Interstate 90 East. Take Interstate 90 East to State Route 91, also known as S.O.M. Center Road (exit #189 immediately to your right). Coming from the North & East:
Take State Route 2 West to State Route 91 (also known as S.O.M. Center Road). Go South (right turn) on State Route 91 to Euclid Ave. (otherwise known as State Route 20). Go East (left turn) on Euclid Ave. to Lake Hospital on the South side of the street (on your right). The office is in the Physicians Pavilion behind and on the East side of the hospital.
