Insurance and Procedures
We participate in most major insurance plans, including Medicare, Medical Mutual, Aetna/US Healthcare, QualChoice, Cigna, United Healthcare, Emerald Health, Lubrizol, PHCS, Anthem and many others.
Payment for a hearing aid is due when the hearing aid is delivered. We will bill your insurance company if you have hearing aid coverage. We will either have your insurance company send you the payment or we will forward you the payment once we receive it.
Please print the Insurance Coverage Acknowledgement form, sign and bring with you to your first appointment.
